Prom Fair 2010 Registration

Location: Rochester Riverside Convention Center

* Please read all eligibility requirements before filling out this form.

You must be a high school junior or senior, actively attending school and attending your own prom.

An invitation and a valid school id are required to participate in Prom Fair 2010.

Please be prepared to wait, it is 1st come, 1st served, we can only accommodate so many girls at one time.

You may bring one guest and may shop for one dress and matching accessories.

The donation for a dress is $5, accessories are $1 and $2. Cash only please.

Please fill in all fields.

All applications are reviewed as they are received.

Once they are approved an Email invitation to Prom Fair 2010 will be sent to the address provided. Please print it and bring it with you to the prom fair.

Please provide us with a working email address to send your Invitation for Prom Fair 2010. If you do not have an email address, please make sure your reference has one.

Please provide us with a workin phone # where we can reach you. If you do not have one, please make sure we have a working # for your school reference.

In order to process your application you must provide a school reference. Please provide the name and contact information of a teacher, counselor or administrator from your school. It is your responsibility to get permission from that person for us to contact them. Also, if they are going to receive your Invitation to Prom Fair 2009, let them know. We must have a working phone number and email address for your reference in order to process your application.
